I'm having no joy registering those .ocx and .dll files, either through the Register Files thing or manually through regsvr32. I'm running a 64bit version of Windows 7, could this be the problem?
Full disclosure:
The engine is Visual Basic 6.
I assume you've already tried running both "client.exe" and "register files.exe" in compatibility mode with say... 2000. Also run as an Admin.
The server is on a Win 7 64bit OS, and works "perfectly".
I hope this fixes your issues with the game.

I have a question as well. I have no problem starting the game, I can log in and other stuff, but the map is all black, and I seem to be the only player in the game. What's happening?
If you're having this issue, please run the Register Files.exe program in your download folder.
Run both the Register Files.exe and client as Admin is you're still having issues.
Looks awesome, but I have one question, what exacly is this game about and what do you do in it before I decide to buy it, even though I will probably buy it anyways.
hmm, i'm trying to play this for the first time but when i try to start it says "Critical Error (545). SilverQuest will attempt to register the required dll/ocx files and will then close." anyone know how to fix this?
Yeah, I'm getting the same problem. No idea why.
